Revised Claim Form
The National Uniform Claim Committee (NUCC) approved changes to the CMS-1500 (08/05) and the UB-04 Health Insurance Claim Forms. The new versions were accepted October 1, 2006.
Beginning January 1, 2008, the older versions of these claim forms will no longer be accepted for payment. Avoid having your claims returned. Submit the new CMS-1500 (8/05) and the UB-04 claim versions now. Better yet, avoid these delays by submitting your claims electronically!
